Online Nursing Credentials Request Form

What is a credential file?

It is made up of supporting documentation for the job search process. Certain fields such as healthcare, require supporting documentation from faculty and clinical supervisors in addition to a typical application and resume submission.


When can I create a file?

It is recommended that students create a file at the beginning of their last year of a program. This allows the file to be set up and the necessary forms completed prior to beginning the job search.


What is included in my file?

Your file consists of a placement data form, which you sign giving PCC permission to share your information with prospective employers and the student reference form that is completed by appropriate faculty or site supervisors.


Who should I ask to complete a reference for me?

It is recommended that you ask professional contacts who can speak to your academic ability, experience in your chosen field, or past work opportunities where your "soft skills" can be highlighted. It's important to ask any reference for a time to talk with them about your career goals and ask them if they would be open to completing a reference for you. Offering to come back and pick up the reference from them can make the process easier for those who complete a reference form.


How many references should I have in my file?

Three references is the preferred number to include.


How long will Career Services keep my credential file?

Three years. 


Who do I contact to request my references be sent to an employer?

To request your references be faxed to an employer, you must visit the Career Services office located in the Goess Student Center, Room 120 or complete the Online Nursing Credentials Request Form at the top of the page.  Phone call requests will not be accepted. 

Online requests for references received prior to 3:00 p.m. will be fulfilled the same day.  Requests received after 3:00 p.m. will be completed on the next business day.  Please note that excluding any information from your request will delay the transmission of your references.
